Athletic Physicals

Athletic physicals for all Hilldale athletes who wish to compete during the 2016-2017 school year will be May 9, 2016 during athletic periods. Athletes will be given documents that must be completed and signed by parents during athletic periods the week of May 2-6. If you need a copy of the forms, please go to the HS or MS office or print from the website under Athletics-->Forms.
