Flu Shots for Students for Staff
Launch the media gallery 1 playerCherokee Nation will be providing flu shots for Hilldale students and staff November 2-3.
Flu shots will be administered at the elementary on November 2, from 9:00 a.m. to 12:00 p.m. and the middle school/high school on November 3, from 9:00 a.m. to 12:00 p.m.
To receive a shot, you must have a registration form and pre-screening tool completed. A printable copy of these forms is below.
