Day Care, Head Start, K-12

Day Care, Head Start- Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at (www.cdc.gov/nip)

K-12- K-7: 5 doses unless 4th dose given after age 4 years; 7-12: 3 doses

Hepatitis A

Kindergarten, Seventh Grade

Day Care, Head Start, K-4, 7-11 - 2 doses with 1st dose on or after the 2nd birthday and 2nd dose 6-18 months later

Hepatitis B

Kindergarten, Seventh Grade, Eighth Grade

Day Care, Head Start - DC, HS: 3 doses or age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip);

K-4: 3 doses; grades 7-12: 2 or 3 doses (based on vaccine used)

Hib

Day Care, Head Start

Day Care, Head Start- Required for ages 1-4, Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip)

Measles, Mumps, Rubella (MMR)

Day Care, Head Start, K-12

M2- K-12 - One MMR required prior to entry, may enter and receive 2nd MMR 28 days after 1st MMR

Day Care, Head Start, K-4 - Required for children in K-4 during the 2002-2003 school year. This requirement will extend up one grade level each year until all grades, K-12, are covered. Parental history of chikenpox disease is accepatable.
